About the role

As Transform NDIS Support continues to grow, we are looking for passionate, empathetic, and experienced Disability Support Workers to join our team. We currently have several casual positions available:

Female and Male Disability Support Workers to join our team across the Southern and Northern Suburbs.

Applicants must be based in the Southern or Northern Suburbs, or be willing to travel.

Minimum 5-hour support shifts available daily.

Opportunities for overnight passive sleepover shifts.

Ideal for reliable, compassionate and experienced Support Workers seeking consistent hours and meaningful work.

Key responsibilities

Provide person-centred support and personal care, including showering, dressing, grooming, toileting and meal preparation.

Support clients with daily living activities, household tasks, shopping, banking, personal administration and maintaining a safe, clean home environment.

Provide emotional and behavioural support, building positive relationships through active listening, empathy and respectful communication.

Support clients with medication routines and healthcare needs, including attending medical, therapy and other healthcare appointments where required.

Promote community participation and social inclusion, supporting clients to access activities, services, hobbies and interests that align with their goals and preferences.

Provide transport and community access support and accompany clients to appointments, meetings and community activities as required.

Maintain accurate case notes, progress records and relevant documentation, while reporting concerns, incidents and changes in client needs promptly.

Work collaboratively with clients, families, healthcare providers and other support workers to ensure consistent, high-quality support.

Promote choice, independence, dignity and positive routines, encouraging clients to achieve their personal and lifestyle goals.

About You

You must have demonstrated understanding of a person-centred approach, good verbal and written communication skills, the ability to connect with others and ability to follow individual support care plans and working procedures.

Skills, Qualification and Experience

Certificate III/ Certificate IV/diploma in Disability and/or Mental Health and/or relevant field.

Have at least 1 year work experience on the same role

Current First Aid & CPR certificate

Current Working with Children Check

Current NDIS Worker Screening Check and NDIS Modules

Current Medication Administration certificate

Valid Manual Handling certificate

Valid Infection Prevention & Control certificate
