About us:
Cairns and Hinterland Hospital and Health Service covers an area of 142,900 square kilometres. We go as far as Tully in the south, Cow Bay in the north and Croydon in the west.
The Cancer Care Centre is located in the Liz Plummer Cancer Care Centre (Block E) on the ground and first floor, adjacent to the main Cairns Hospital campus. The Oncology Day Unit provides ambulatory care for day only out-patients patients, whilst Specialist Outpatient Clinics provide Haematology, Medical Oncology, Radiation Oncology and Palliative Care clinics all which are collocated on the First Floor of the Liz Plummer Cancer Centre (Block E). Radiation Oncology services are provided by Radiation Oncology Queensland (ROQ), and they are located on the Ground Floor of the Liz Plummer Cancer Centre. Queensland Health and ROQ have a shared care partnership for cancer patients requiring radiation services.
About you:
In your new role you will......
- Facilitate continuity of care between the patient, individual health care professionals and health care teams across different settings by establishing timely communication, clear referral pathways and acting as a liaison between family and members of the health care team.
- Perform assessment and screening of clinical and supportive needs for Cancer Care patients at risk of adverse clinical or psychosocial outcomes.
- Providing timely and consistent Cancer Care specific education and information to patients and their families.
- Collaborating with all members of the team ensuring prompt referral to specialist, allied health and support services.

About Cairns and Hinterland Hospital and Health Service
Cairns and Hinterland Hospital and Health Service covers an area of 142,900 square kilometres. We go as far as Tully in the south, Cow Bay in the north and Croydon in the west.
The outer western region of our service includes extremely remote communities. Cairns Hospital is the only major referral hospital in Far North Queensland and also provides care for people from Cape York and the Torres Strait.
We provide most specialised health services. A small number of our high-level acute services are done in Townsville and Brisbane.
