As a Care Coordinator (Healthcare), you will be responsible for the following duties:

Collaborate with the multidisciplinary care team to deliver holistic and patient-centred care.

Empower patients and caregivers to take ownership of their health through the use of technology and digital health tools.

Promote awareness and accessibility of community and healthcare resources to support positive health outcomes.

Build and maintain strong relationships with internal stakeholders, patients, and caregivers.

Support preventive health initiatives by improving screening and vaccination uptake.

Gather and document relevant social determinants of health to support care planning and interventions.

Assist in coordinating and delivering patient care activities using IT systems for tracking, documentation, and monitoring clinical indicators.

Perform basic health parameter measurements and support preventive care programmes.

Provide basic health education on lifestyle management, physical and social activities, and self-care practices.

Guide patients on the use of home health monitoring devices such as blood pressure monitors, glucometers, and weighing scales.

Advise patients on recommended vaccination requirements and national health initiatives.

Any other ad hoc duties as assigned.

Requirements:

Possess a Diploma in Health Sciences (Health Management) or other related field.
