Primary Objectives of Position

Provide direct casework management to clients in AWWA Senior Care Centre and cross-coverage for clients in AWWA Rehab and Day Care Centre.

Develop and implement psychosocial programs and caregiver support groups for identified projects in AWWA Rehab and Day Care Centre and AWWA Senior Care Centre.

Strengthen social work function in AWWA Health & Senior Care, such as aligning cross-department social work processes and work in collaboration with other agencies and ministries where needed, to address service gaps in the community and advocate for clients and caregivers.

Job Responsibilities

Assess and coordinate new referrals and conduct needs and intake assessments.

Perform psychosocial assessments, collaborate with the multidisciplinary team to deliver person-centered, holistic care plans, and track client progress via 6-monthly (day care) reviews.

Prepare timely social reports for new admissions and maintain accurate, up-to-date client records and documentation in compliance with organizational and data protection requirements.

Administer and review Household Means-Testing (HHMT) processes, and assess eligibility for financial assistance schemes (e.g., Medifund, deviation cases, Medisave), ensuring accuracy, timeliness and audit compliance.

Design, implement and evaluate psychosocial programmes, group work and caregiver support initiatives based on identified client and community needs.

Build rapport and professional relationships with clients and significant others (e.g., spouse, children, siblings and partner etc.).

Establish and maintain collaborative relationships with community partners and relevant agencies to enhance service delivery and support for clients and caregivers.

Conduct case closure upon achieving sustainable service outcomes for clients and their families with appropriate support systems in place.

Provide guidance and support to Social Service Assistants in casework management and service delivery.

Contribute to the advancement of social work practice within Health and Senior Care through training, knowledge sharing, and participation in internal and external workgroups.

Participate in continuous work improvement/ quality improvement projects as required by the organisation.

Collate, analyse and submit accurate client data and service statistics in a timely manner to meet organizational and stakeholder reporting requirements.

Respond to crisis situations in accordance with organizational policies and standard operating procedures.

Ensure adherence to service standards, programme requirements, audit processes, and prevailing personal data production guidelines in all aspects of work.

Requirements

Recognised Degree in Social Work and Full registration with Singapore Association of Social Workers (SASW)

Knowledge of social work ethics, case management frameworks, and relevant legislation (e.g., PDPA).

At least 2 to 5 years of experience with social work in community care sector

Understanding of the geriatric landscape in Singapore

Competent in BPSS assessment, case management, and intervention planning (including mitigating risks at client’s level) & practice evaluation

Excellent communication and counselling skills with elderly clients, their caregivers and with multidisciplinary professionals.

Ability to maintain confidentiality and professional boundaries in line with SASW Code of Ethics

Ability to draw and apply relevant theories in direct case work for appropriate intervention/ care plans

Ability to identify, profile and prioritize clients based on needs evaluation

Proficient in MS Word, Excel and PowerPoint and capable of maintaining clear and timely case documentation and reports.
