As an Assistant Nurse Clinician (Home Care), you will be responsible for the following duties:

Provide clinical supervision, mentorship and coaching to nursing staff to ensure the delivery of safe, high-quality and evidence-based patient care.

Support the triage and prioritisation of referrals, and contribute to case discussions to promote timely and appropriate care planning.

Participate in departmental and multidisciplinary meetings, while providing support with operational and administrative responsibilities.

Manage clinical escalations raised by nursing staff by providing appropriate clinical guidance, advice and support.

Monitor nursing practices, workload allocation and resource utilisation, and identify opportunities to improve efficiency and service delivery.

Contribute to the development, review and implementation of community nursing guidelines, clinical protocols and standard operating procedures.

Support staff performance management by identifying training and development needs, facilitating competency enhancement and encouraging continuous professional development.

Participate in quality improvement activities, clinical audits and service-related projects to promote positive clinical outcomes and enhance patient safety.

Undertake any other duties and responsibilities as assigned by the reporting officer.

Requirements:

Possess a Degree in Nursing with a valid full registration with Singapore Nursing Board (SNB).
