Community Health Practitioner - Aged Care

Duration: Part time, fixed term until 30 June 2027

Work pattern: 4 days per week or 30.4 hours across 5 days – flexible with applicant

Base Salary: $100,624 - $106,783p.a. as per AHP 4.1-4.4 / $105,787 - $113,029 p.a. as per RN 3.1-3.3

About the role

Danila Dilba Health Services delivers flexible, culturally safe aged care services to Aboriginal and Torres Strait Islander elders in the Yilli Rreung (Greater Darwin) region. The program aims to improve access, quality, and community connection in aged care, empowering elders to lead decisions about their support and services.

The Community Health Practitioner – Aged Care works within a multidisciplinary team and requires a commitment to delivering care aligned with the principles of Social and Emotional Wellbeing (SEWB), to ensure Aboriginal and Torres Strait Islander elders receive holistic, person-centred, trauma-aware and healing-informed care that meets the Strengthened Aged Care Quality Standards, Statement of Rights, Aged Care Code of Conduct, and Danila Dilba Health Service (DDHS) values, policies and procedures. The role also provides short-term clinical leadership and operational continuity during periods of leave of the Clinical Nurse Consultant (CNC) Aged Care.

Responsibilities:

As the Community Health Practitioner Aged Care, you will play a key role in addressing the unique needs of the Elders in our care by strengthening the connection between our aged care and clinic services and ensuring continuity of care. Working within a strengths-based and culturally grounded framework, you will undertake clinical assessments, develop and review care plans, provide health education, and advocate for Elders within the community aged care setting. Through this work, you will help ensure that care and service plans reflect each Elder’s current needs, goals, preferences, and overall wellbeing.

Clinical Assessment and Care Coordination:

Provide best practice, evidence-based assessment and clinical care coordination according to CARPA guidelines,

Demonstrate commitment to individualised care, and evidence-based best practice standards within scope of practice, which facilitates choice and control and maximises independence.

Ensure the maintenance of electronic health records and care planning, which reflects appropriately planned care in compliance with the Strengthened Aged Care Quality Standards, Statement of Rights, Aged Care Code of Conduct and legal requirements.

Respond to clinical changes in elder conditions and initiate intervention and consultation with members of the DDHS multidisciplinary team and external services to facilitate care and follow up of clients to support optimal health outcomes.

Address any new health or clinical needs, determining how these will be supported, and outlining the process to respond to deterioration or medical emergencies.

To be successful you will need:

Essential

Current AHPRA registration as a Registered Nurse or Qualification as an Aboriginal Health Practitioner

and current unrestricted registration with AHPRA.

Min 5 years clinical experience in aged care, community nursing, palliative care or chronic disease management, including clinical assessment and care coordination.

Understanding of Aboriginal and Torres Strait Islander culture with a commitment to culturally safe, person-centred care.

Experience in managing high-impact risks, including falls, medication safety, and infection control.

Proficiency in clinical documentation and maintaining accurate electronic health records.

Commitment to continuous improvement

Desirable

Ability to mentor and support junior staff

Knowledge of Aged Care Quality Standards, clinical governance, and regulatory requirements in aged care.

What we offer:

6 weeks annual leave per annum (pro-rata)

5 days paid leave during end of year shutdown (on top of annual leave).

Salary packaging of up to $15,990 per annum.
