Care Manager - Home Care & Community Services

We are seeking an experienced, compassionate and highly organised Care Manager to join a growing home and community care team.
This is an exciting opportunity for someone who is passionate about delivering exceptional client outcomes, building strong relationships and ensuring people receive high-quality, person-centred care in their own homes.
Reporting to the Regional Care Manager, you will take ownership of a portfolio of clients, oversee the quality and coordination of their care, support a team of carers and nurses, and build strong relationships within the local community.

About the Role
As a Care Manager, you will play a key role in ensuring clients receive safe, reliable and personalised care that supports their independence and quality of life.
You will work closely with clients, families, carers, Registered Nurses and other stakeholders to coordinate services, develop appropriate care plans and proactively address issues as they arise.
Beyond day-to-day care management, you'll also have the opportunity to build relationships within the community, attend networking events and contribute to the continued growth and improvement of services.
Your key responsibilities will include:
- Managing and coordinating the delivery of high-quality care to clients within your portfolio
- Conducting client home and care assessments
- Developing, reviewing and maintaining individualised client care plans
- Conducting regular client reviews and check-ins to ensure services continue to meet their needs
- Building trusted relationships with clients, families, carers and other stakeholders
- Supervising care delivery and maintaining high standards of service
- Supporting and resolving client and carer concerns promptly and professionally
- Conducting orientation visits with new clients and carers
- Supporting the recruitment and onboarding of carers and Registered Nurses
- Managing and reporting incidents and hazards appropriately
- Ensuring services are delivered in accordance with relevant legislation, regulations, policies and quality standards
- Developing relationships with community organisations, referral partners and other stakeholders
- Attending networking events and identifying opportunities to grow the organisation's community presence
- Supporting new client enquiries and helping prospective clients transition into services
- Contributing ideas to continuously improve systems, processes and the overall client experience
- Identifying innovative solutions to meet changing client needs
About You
We're looking for someone who genuinely cares about people and understands that excellent care management is about more than coordinating services - it's about building trust, listening to clients and ensuring they feel supported.
You will be confident managing competing priorities, communicating with a wide range of people and taking ownership of issues through to resolution.
To succeed in this role, you will ideally bring:
- Experience within home care, aged care, disability, community care or a related health/community services environment
- Strong client relationship and stakeholder management skills
- Excellent communication and interpersonal skills
- Strong organisational and time-management abilities
- Confidence managing client and workforce-related issues
- A proactive and solutions-focused approach
- The ability to work independently while contributing positively to a wider team
- A strong understanding of person-centred care and quality service delivery
- Confidence networking and developing relationships within the community
- A commitment to compliance, quality and continuous improvement
- A valid Driver's Licence
Desirable Qualifications
The following qualifications would be advantageous:
- Certificate IV in Training
- Certificate IV in Community Care
- Graduate Diploma in Occupational Health and Safety
- Other relevant qualifications within aged care, community services, nursing, health or care management
