360 Health + Community is looking to welcome our inaugural Senior Navigator/Practice Lead for our Peel Youth Mental Health Navigation and Coordination Service (to be known as 'Peel Youth Pathways')!
360 Health + Community is an established, experienced and growing not-for-profit organisation delivering specialised mental health services, headspace (x3), suicide prevention, psychosocial support, chronic health & aged care, allied health, Street Doctor, and NDIS support coordination and allied health services. We operate throughout the Perth metropolitan area, the Mid-West (out of Geraldton), and the Goldfields (out of Kalgoorlie). In delivering our NDIS support coordination and recovery coaching services we specialise in the psychosocial domain and work with complex participants, including those living with more than one disability.
Our youth participants (12-25 years of age) encompass a rich range of diversities – culture, language, religion, CaLD, Aboriginal and Torres Strait Islander, gender, sexuality, living circumstance, socio-economic status, educational and economic participation, and health status – and can include those with guardians, administrators, and child protection and family services.

About the role – Senior Navigator/Practice Lead
The Peel Youth Mental Health Navigation and Coordination Service (PYMHNCS) is a new, innovative program and the inaugural Senior Navigator/Practice Lead will play a significant role in implementing, supervising and guiding the outreach, support and connection services to the youth clients of this program. The Senior Navigator/Practice Lead will work within a multi-disciplinary team of psycho-social service navigators supporting youth who are experiencing mental health conditions and biopsychosocial complexities. This program will work in concert with the Wandjoo Gateway, and a significant part of this role is ensuring collaboration and integration with the network to achieve improvements in the mental health care of youth.
Key aspects of this role:
- Work with the Team Leader Peel Youth Pathways to coordinate the implementation and delivery of the PYP program.
- Work within the PYP team to provide client assessment, planning, and stakeholder engagement to young people.
- Oversee the initial screening and assessment of young people who access PYP, including referral on to appropriate service where necessary e.g., in a high-risk situation.
- Create an environment and relationships with clients whereby young people are actively encouraged to participate in decisions relating to their own care.
- Be supportive of an integrated care approach through agency liaison, stakeholder engagement, and shared of understanding of young people who have complex issues.
- Supervise PYP navigators to ensure performance against agreed objectives and standards and support the performance of PYP staff.
- Ensure PYP staff receive practice supervision.
What we are looking for / being successful in the role, you will have:
- Possession of tertiary qualifications in Psychology, Social Work, Nursing, Counselling, Occupational Therapy, or other allied health discipline recognised by the tertiary mental health services sector or equivalent experience.
- Eligible for registration or membership with appropriate peak professional body, such as AHPRA, AASW, PACFA, ACA.
- Minimum of 2 years’ experience in direct service provision to young people and their families through a variety of programs and initiatives. This would include a strong understanding of how to work appropriately and effectively with young people aged 12 to 25 years.
- Demonstrated experience of psychosocial assessment of clients and referral pathways through a mental health or similar service including referrals to and liaising with tertiary services.
- Experience providing clinical or practice supervision within a mental health setting.
- Demonstrated understanding of the issues and factors that influence the mental, physical, social, and emotional well-being of young people aged 12 to 25 years of age and their families. This would also include an awareness of the range of professionals that work within, or meet, the youth health sector.
- Demonstrated experience and ability to work within and contribute to the development of clinical systems for a health or community services organisation, including effectively responding to clinical risk, and coordinating a team.
- Strong demonstrated experience in managing risk with clients, directly or indirectly, including knowledge of risk assessment and safety planning.
- Current WA Driver’s License and access to a roadworthy, comprehensively insured vehicle.
- National Police Certificate, Working with Children Check
360 Health + Community offers:
An attractive remuneration package based on:
- $44.51 to $50.61 per hour PLUS superannuation, PLUS salary packaging.
Starting rate based on the qualifications and experience you bring to the role. Appointments are usually made at the starting rate for the band.

The base of operations will be our Mandurah office. The role on offer is full-time and fixed-term to 30 June 2027.
