About the role

Youth Prevention and Recovery Care (YPARC) is a short stay residential service, delivered in partnership with Victorian hospital networks and Health Service, to support young people aged 16-25 years who require additional support to manage their mental health. The short stay Adult ‘step up, step down’ Prevention and Recovery Care (APARC) residential service, also delivered in partnership Victorian hospital networks and Health Service, supports clients aged 25-65 years with psychosocial disability to transition from an acute hospital setting or prevent hospital admissions for those entering directly from the community.

The YPARC and APARC services provide individualised 1:1 support, shared supports and group work based on client’s strengths to enable them to achieve their recovery goals, enhance relationships with families and carers, develop everyday living and social skills, build community networks, and increase confidence and capacity to live safely in the community.

We are looking for three compassionate and self-motivated Community Mental Health Practitioners for three opportunities (2x Perm, Part Time positions at 31 hrs and 20 hrs per week, and 1x Casual position) to join our supportive team in the Frankston area. These roles operate on a 24/7 rotating roster including active night shifts, and may move between our Frankston sites to support service needs. You will provide intensive person-centred, recovery-oriented supports to clients to achieve their wellbeing goals, build capacity and daily living skills, strengthen social and community connections, and live independently in the community.

Key responsibilities

  • Deliver high quality services and facilitate positive educational and social experiences.
  • Support clients to return to their community, live independently and enjoy a meaningful life through a staged approach to recovery.
  • Provide information and mentoring to clients to establish relationships with families and carers, create community networks and access to relevant services.
  • Plan and deliver group programs that aid clients to build their skills and focus on recovery.
  • Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to client’s recovery goals.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
  • Proven capability to implement therapeutic, recovery oriented frameworks and practices including risk mitigation, trauma informed care and family inclusive practice.
  • Relevant experience in facilitating group activities for clients experiencing mental ill health.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
  • Current First Aid and CPR certifications and ability to perform these duties as required as part of the role.
