Dependable Care DRPS provides disability supports across Victoria, Queensland and Tasmania. Dependable Care DRPS is a registered NDIS, TAC and Workcover provider. Dependable Care DRPS provide Support Coordination and Psychosocial Recovery Coaching.

We are committed to providing an individualised, meaningful and person centered service. We specialise in providing services to participants with psychosocial disabilities and specifically people that experience homelessness, lack of line of support and in need of a support network to help them navigate through tough times.

About the role

You are expected to support and promote community development initiatives that enable our residents and clients to overcome barriers and gain equitable access to essential services, social participation, and opportunities in all aspects of life.

Tasks & Duties

· Engage with clients to identify their needs in areas such as health, accommodation, recreation, employment, social participation and daily living.

· Conducting further interviews to identify the barriers and assess the extent to which they affect our clients.

· Plan, develop and implement corresponding solutions and supports within our service environments.

· Providing guidance and practical support to help individuals address challenges.

· Following up and record clients’ progress and keeping management team updated.

· Referring clients to professional support providers like Skills and Jobs Centres, Language Schools, Recreation Centres.

· Assessing the overall community needs within Dependable Care and identifying opportunities to enhance our accommodation, health, recreation, daily living, and social engagement supports and resources.

· Collaborating and Liaising with external stakeholders including government and non-government agencies as well as NDIS commission to discuss our community issues.

· Let residents and clients understand the types of support available to them and how to access these community services effectively.

· Developing funding proposals and progress reports to ensure ongoing support from NDIS commission, local councils and other relevant stakeholders.

Skills & Experience requirements

· AQF Associate Degree, Advanced Diploma or Diploma in Community Service related areas are preferred.

· At least 1 year relevant work experience.

· Highly developed communication, empathy and relationship-building skills.

· Working With Children’s Check

· NDIS Workers Screening Check
