Practice Nurse

The Hub Medical Centre Burpengary is a GP-owned and operated medical practice with deep roots in the community. We pride ourselves on delivering high-quality, compassionate care to patients across the Moreton Bay region.

Our well-established, friendly team includes nine GPs, supported by allied health professionals, including psychologists, physiotherapists, podiatrist, dietitian, and visiting consultants. We have a great supportive team and we are wanting that special person to come and join our Hub Nursing family in a permanent part-time position. If you are a Division 1 Registered Nurse, and you would like a fast paced role where no two days are the same, we would love to hear from you.

The Opportunity

You’ll work across our busy treatment room and chronic disease management service, supporting GPs and patients to manage acute and chronic conditions with compassion and professionalism.

This is a part-time role within a well-supported nursing team, offering a genuine work–life balance in a caring and inclusive workplace.

Your Key Responsibilities

Conduct health checks and assessments to identify patients at higher risk of chronic disease or hospital admission.

Prepare GP Management Plans (GPMPs) and Team Care Arrangements (TCAs).

Support patients in achieving their health and wellbeing goals through education and coaching.

Work collaboratively with GPs to promote preventive health and early intervention.

Provide clinical care within our busy treatment room, including wound care, skin cancer procedures, immunisations, ECG, ABI, and spirometry.

Use and maintain accurate clinical documentation and software (training provided for Best Practice and our CDM systems).

What We’re Looking For

AHPRA registration – Registered Nurse preferred.

Ideally 1 - 2 years of experience in CDM within general practice.

Familiarity with Best Practice software (preferred, not essential).

Excellent communication and teamwork skills.

The ability to work autonomously while contributing positively to a team environment.

A genuine passion for patient care and continuous learning.
