About the role:
Hunter Primary Care (HPC) is seeking an experienced and compassionate Respiratory Nurse Care Coordinator to join our dedicated team. This temporary part-time opportunity is part of our Multidisciplinary Team Pilot Program, delivering patient-centred care to clients with chronic respiratory conditions for a temporary period of time.
Working collaboratively with GPs, allied health professionals, and community services, you will coordinate comprehensive care, helping clients to access the right supports and improve their health, wellbeing, and independence.

Key responsibilities:
- Provide holistic, client-centered care coordination for people with chronic respiratory conditions
- Conduct assessments including home visits as required
- Support clients to identify goals and develop strategies for long-term health management
- Promote and strengthen partnerships between clients, GPs, and health service providers
- Maintain accurate and timely documentation in line with HPC policies and program deliverables
- Actively arrange and participate in multidisciplinary meetings and contribute to integrated care pathways
About you:
To be successful in this role, you will have:
- Current AHPRA registration as a Registered Nurse
- Minimum 2 years’ experience in respiratory nursing or chronic disease management
- Strong understanding of person-centred practice and capacity building approaches
- Excellent communication and interpersonal skills with the ability to engage diverse stakeholders
- Strong time management and organisational skills with the ability to manage a complex caseload
- Proficiency in Microsoft Office and clinical data systems
