About the Role
We are seeking a proactive Registered Nurse to work across our health assessment services and treatment room. This role goes beyond reactive support, focusing on care coordination, chronic disease management, health coaching, and Chronic and Complex Care (CVC).
Candidates must have a minimum of 3-5 years of nursing experience to be considered for this position.
Essential prior experience in general practice or primary care settings is required for this role.
Current AHPRA registration is required at the time of application.
This is a full-time position.
Key Responsibilities
Treatment Room Duties (including but not limited to):
Triage, resuscitation, and patient monitoring
Immunisations
Wound care
ECG and spirometry
Assisting with minor procedures
Sight testing
Health screening for new patients
Cold chain monitoring
Managing urgent recalls
Health Assessments (including but not limited to):
Conducting assessments for patients aged 75+ and 45–49
Preparing documentation and liaising with providers
Supporting patients with health coaching and chronic disease management
Contributing to chronic and complex care (CVC) planning and follow-up
