We're seeking an experienced Support at Home Case Manager to join a small, supportive team in Mornington. This is a full-time locum contract with the potential to transition into a permanent position if it's the right mutual fit.

What You'll Be Doing

Managing a caseload of approximately 30-40 clients

Coordinating care plans and providing individualised support

Onboarding new referrals and conducting assessments

Using MAC (My Aged Care) confidently to manage client journeys

Working collaboratively within a close-knit team environment

What You'll Bring

Solid Case Management experience within aged care, community care or home care

Strong MAC platform knowledge

Experience onboarding and managing new clients

A client-first approach with excellent communication skills

Role Details

Location: Mornington-based (office + community) with WFH 1 day per week once confident in the role.

Start: ASAP

Hours: Full-time

Contract: Locum with view to permanency
