About the position
The Program Leader Healthy Communities is responsible for providing effective operational management and leadership to staff as they provide a broad range of primary health care, health promotion, and community support services to the people and communities Grampians Community Health serves. This position will entail travelling between GCH sites.
Programs within Healthy Communities currently include Health Promotion, Community Health Nursing, (including general, women’s health, movement disorder, diabetes and chronic conditions) Amplify (formerly FReeZA), Pathways to Good Health, Community Paramedic, and the Healthy Mothers, Healthy Babies program. This program leader also leads the Social Support Recovery Relief and Recovery response.
As a key member of the leadership team, the Program Leader Healthy Communities is part of a cohort committed to developing a company culture focussed on excellence in service standards, continuous improvement and quality assurance.
The position will contribute to achieving the vision, goals and values articulated in the organisation’s strategic plan.
Skills and experience
• Tertiary qualifications in a health, social welfare or business discipline.
• Proven industry experience of two years in health, social welfare, community development or direct care roles.
• Strong understanding of and commitment to the social determinants of health and how to apply these across the community health sector.
• Strong understanding of continuous quality improvement and accreditation.
• Awareness of government policies and legislative requirements relevant to the community health sector.
