About the role

Permanent part time opportunity to provide advanced clinical expertise and care relevant to the diabetes education. The Credentialled Diabetes Educator (CDE) provides direct and indirect care as required and works in collaboration with the Nurse Unit Managers, Nurse Educators and wider multidisciplinary health care team to support the clinical units in provision of safe quality healthcare through exemplary clinical nursing standards of practice for the specialty of diabetes nursing.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Manage an allocated workload to deliver high level diabetes education to patients across the continuum of care, within a multidisciplinary team, in accordance with prescribed professional and ethical standards, utilising a high degree of independence within the work unit, including application at a HHS and state-wide level, with infrequent need for direct clinical practice supervision.
  • Provide clinical advice to the multidisciplinary team, working cooperatively and effectively within the team and with patients/clients/families to provide high quality patient care.
  • Identify, monitor and report on clinical practices and outcomes within clinical area and actively participate in initiating, developing and implementing quality and service improvement activities that enhance the delivery of diabetes services and patient outcomes.
