Your Next Opportunity

K-Life is seeking a compassionate and skilled Community Nurse to deliver high-quality clinical care and care planning for our community clients.

You will be responsible for assessing, coordinating, and providing direct clinical care to clients receiving Home Care, NDIS, Palliative Care, CHSP, and private services.

This role is hands-on, working directly with clients to improve their health, independence, and overall well-being.

Key Responsibilities

Deliver direct nursing care across home care, disability, palliative care, and community health.

Develop and implement care plans, ensuring clients receive appropriate clinical support.

Conduct assessments, chronic disease management, wound care, and medication administration.

Coordinate client care services, arranging allied health and specialist referrals as needed.

Work closely with the Health Navigator, ensuring best-practice clinical care is delivered.

Liaise with clients, families, and carers, ensuring a holistic, person-centred approach.

Maintain accurate clinical documentation, meeting compliance and funding requirements.

What We’re Looking For

✔ Registered Nurse (AHPRA registration) or equivalent health-related qualification.
✔ Experience in community care, home care, or primary healthcare nursing.
✔ Strong care planning and assessment skills, with a focus on patient-centred care.
✔ Excellent communication and relationship-building skills to engage with clients and families.
✔ Ability to work autonomously, managing your caseload efficiently.
✔ Current First Aid Certificate, CPR, and unrestricted Australian Driver’s Licence.
