Registered Nurse – Patient Care Manager

About the Role
We’re looking for a compassionate and experienced Registered Nurse to join our growing telehealth team supporting Greencare, Nuleaf, and our Weight Loss Program.

This fully remote role is focused on patient care, proactive follow-ups, and delivering unlimited free nurse consultations to support treatment journeys. You’ll play a vital role in ensuring patients feel supported, informed, and connected throughout their care experience.

Our technology ecosystem is fully integrated for efficiency and ease of use. We operate Best Practice as our management system, AutoMed as our booking engine, and ERX for electronic scripting — all working together seamlessly through our remote server environment.

Key Responsibilities

Patient Consultations & Care

Conduct telehealth nurse consultations and structured check-ins for new patients.

Deliver unlimited free consultations for Weight Loss Program patients.

Provide timely follow-ups for Greencare and Nuleaf patients.

Educate patients on treatment plans, safe use of medications (including medical cannabis), and care pathways.

Assist patients with bookings, telehealth access, and navigating their treatment journey.

Escalations & Clinical Management

Act as a clinical point of contact for patient concerns, side effects, or escalations.

Liaise with practitioners on patient progress, treatment tolerance, and formulary options.

Escalate cases appropriately to practitioners or the clinical lead.

Maintain accurate, compliant clinical notes for all interactions.

Team & Practitioner Support

Work closely with doctors, pharmacists, and patient care coordinators.

Support smooth communication and workflow across clinics.

Ensure patients receive accurate, up-to-date information about treatments and medications.

Contribute to refining patient engagement processes and clinical communication.

About You

Current AHPRA registration as a Registered Nurse.

Strong communication and patient engagement skills.

Experience in telehealth, chronic care, or general practice (highly regarded).

Ability to manage multiple virtual consultations efficiently.

Strong organisational and documentation skills.

Comfortable discussing treatment pathways and medications with both patients and practitioners.
