As a Clinical Nurse Consultant/Clinical Midwife Consultant in SESLHD you will play a pivotal role to oversee the new Community Supportive Care Services model (CSCS). You will triage the patients to the appropriate community or outpatient supportive service using the established guidelines and triggers. Once accepted to the CSCS, nurses will support case coordination within the multidisciplinary (MDT) team and community healthcare providers, including running a weekly MDT of patients reviewed. Within this role there will be a requirement to undertake clinical reviews across clinics, community and virtual care settings either alone or in conjunction with a Palliative Care Specialist.
The position will also be responsible for providing telephone support to GPs within business hours, for symptom management of patients in the local area, under the supported supervision of the Palliative Care Specialist, as required. In addition to the above, the position will also lead education and research activities and will be required to undertake strategic planning and capacity building activities for the new service.

Selection Criteria:
1. Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse/ Midwife and current NSW Driver’s License and willingness to travel as required in role.
2. Having at least 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the specialty field. In addition, must have approved postgraduate nursing/midwifery qualifications relevant to the field in which he/she is appointed.
3. Demonstrated ability to apply clinical expertise in a consultant role and the ability to work independently.
4. Proven clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a demonstrated commitment to excellence in nursing practice.
5. Demonstrated evidence of the use of critical thinking as it relates to the delivery of advanced evidenced based nursing care.
6. Demonstrated experience in leading and implementing change, and developing direction in clinical practice by initiation, utilisation, review and translation of evidence-based research into practice, within a person-centred framework.
7. Demonstrated effective interpersonal skills, excellent written and verbal communication skills, advanced computer skills and use of software programs to analyse data and produce reports.
8. Demonstrated ability to contribute to strategic and clinical service planning.
