Your Role as our Team Leader
Our mission is to support children, and their families navigate complex needs and issues with a person-centre and ethical approach. As the Team Leader, you will support the team by:
Facilitate monthly panel meetings including referral process, setting agendas, organising and scheduling meetings, distributing minutes, supporting case plan development and follow through of plans with panel members.
Develop and or enhance paradigm where integration and collaboration is committed to, understood and practised by each organisation/department involved.
Provide accurate case notes, report writing, and evaluations as needed.
Develop, manage and evaluate projects to facilitate continuity of care for young people with complex needs.
Identify gaps in service provision and develop protocol for these to be addressed and promote CNAP Panels throughout network using effective communication and marketing plans.
Assist Alliance in broadening funding base to sustain resources including writing formal funding submissions.
This role is a part-time position consisting of 36 hours per week.
What We’re Looking For
Minimum Bachelor level qualifications in social work, human services or related field.
Extensive experience working with “at risk” children, young people and families experiencing complex needs.
Extensive experience using case management principles within a best practice framework.
Team Leadership experience preferably within in a human services related role.
Ability to develop and maintain effective partnerships across the service system to better meet client needs
Essential Requirements
Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)
Relevant probity checks required by legislation and WMQ Limited policy.
Current Driver’s Licence and willingness to travel for work
