At Central and Eastern Sydney Primary Health Network, we focus on improving the health and wellbeing of the people who live and work in Sydney, from East to South and the Inner West. Primary health refers to the healthcare provided in the community, such as a local doctor, pharmacist or psychologist. We provide support and training to these health professionals. PHNs are not-for-profit organisations. We receive money from the government to provide health services that are right for the people in this area. We also work with hospitals, specialists, aged care facilities and others to improve people’s experience when needing healthcare. The Population Health and Chronic Disease Manager is responsible for overseeing primary health care initiatives including sexual health, child and youth health, maternal health, domestic and family violence and disease prevention amongst priority groups.
Reporting to the GM Primary and Community Care, this role will lead the implementation of specific priority areas consistent with the annual planning cycle and in accordance with the funding contract and service level agreements. The role will work closely with internal organisational streams to provide effective leadership and enhance the capability and performance efficiency, efficacy and success of Central and Eastern Sydney PHN (CESPHN). This role will also be responsible for commissioning and providing high level management of projects to ensure key contracted outcomes are met.
The Population Health and Chronic Disease Manager is also responsible for managing and supporting the Population Health and Chronic Disease team to effectively deliver on Primary Care Improvement’s organisational outcomes in a positive, engaging and productive team environment.
The salary range for this role is $133,250 - $141,963 full time equivalent (FTE) and is negotiable based on skills, qualifications and experience. This salary will be prorated according to hours worked.
Successful candidates will require a compatible smartphone for Multi-Factor Authentication to securely access CESPHN systems.
Key Responsibilities
Manage the Population Health and Chronic Disease team by providing direction, clarifying operational priorities, monitoring and reporting on financial and business performance, supporting continuous improvement and delivering on change initiatives.
Support the Population Health and Chronic Disease team in delivering on the team's operational and individual work plans by providing them with professional advice, feedback, and coaching.
Support the General Manager in the application of change management, strategic planning, risk management, policy review and providing expert advice as required in a collegiate manner.
Support development of strategic relationships and partnerships with relevant stakeholders across the CESPHN region including peak health and community organisations, regional GP networks, allied health and community networks, government and private health sectors.
Contribute to a safe, productive and engaging stream working environment by applying CESPHN's people policies and procedures within the Population Health and Chronic Disease team and actively supporting team member's professional development.
Selection criteria – essential
Tertiary qualifications in a health related/management field with expertise in population health and a minimum five years of relevant experience.
Demonstrated experience in project management with the ability to deliver and lead teams in challenging environments.
Demonstrated experience in outcomes-based service commissioning and contract management to ensure key deliverables are achieved.
Strong organisational skills and the ability to set priorities, work effectively, and achieve desired outcomes within specific timeframes.
Proven ability to think conceptually, analyse complex issues, develop innovative solutions within organisational constraints, and prepare high level reports and budgets.
Demonstrated ability to effectively engage a wide range of stakeholders including the local health districts, non-government organisations, universities and primary care clinicians to achieve effective primary health care system reform.
Proven high level written, verbal and electronic communication skills, and ability to successfully manage communications across a decentralised organisation.
Selection criteria – desirable
Relevant post-graduate health qualifications, such as public health, health policy, health management, health sciences or health administration.
Demonstrated experience in horizon scanning and anticipating emerging trends in primary health care.
Sound ability to work with a range of different health professionals with a strong capacity to influence and manage change within a dynamic environment, including the promotion of evidence-based practice.
Demonstrated understanding and ability to identify government health priorities and implement and evaluate primary health care programs.
An understanding of the primary health care environment and experience in communicating with health care professionals and consumers.
Experience working for a Not-for-Profit organisation.
