Nurse Practitioner - Full Time

Sydney Location

We are seeking a highly skilled and compassionate Nurse Practitioner to join a collaborative healthcare team providing innovative care across the community. This role offers a mix of on-site clinical work, telehealth consultations, and visits to residential aged care facilities, allowing you to make a real impact in improving patient outcomes.

The Opportunity

• Deliver advanced clinical assessments, diagnosis, and treatment planning within your Nurse Practitioner scope
  
• Provide care for patients both in clinic and through outreach programs, including aged care facilities
  
• Collaborate with GPs, allied health professionals, and external providers to ensure continuity of care
  
• Use digital health and telehealth platforms to support patients and contribute to quality improvement initiatives
  

What We're Looking For

• Current Nurse Practitioner endorsement with AHPRA
  
• Postgraduate qualifications in advanced practice nursing
  
• Minimum 3 years' clinical experience, with aged care experience highly regarded
  
• Strong skills in communication, leadership, problem solving and patient care planning
  
• Current driver's licence
