Nurse Practitioner - Autonomous Practice | Permanent Role | Up to $155K + Super Clarence Valley Region

About the Role

Reporting to the Clinical Director, you will provide holistic, end to end care within a nurse led primary health model

Ensures seamless continuity of care across acute, chronic, and preventive services

Develops and oversees chronic disease management programs such as diabetes, asthma, and cardiovascular care

Builds strong community partnerships with hospitals, aged care, and local health networks

Essential Criteria:

Current registration as a Nurse Practitioner with AHPRA.

Demonstrated clinical experience in primary health, chronic disease, or urgent care

Proven ability to practise autonomously and within a multidisciplinary team

Strong communication, leadership, and organisational skills

Commitment to accessible, patient-centred care and rural health equity

Skills & Experience:

Minimum 5+ years of experience as a Nurse Practitioner is required with AHPRA

Broad experience in multiple facets - such as women's health, diabetes management and preventative health

Familiarity with Medicare item optimisation for NP models

Interest in health innovation, data-driven care, and nurse-led service design

Demonstrated clinical experience in primary health, chronic disease, or urgent care

Permanent position with full entitlements of (sick and annual leave)

Competitive salary: up to $155,000 pa + super ( pro-rata, depending on hours worked and experience

Options for part-time or full-time hours (20–40 per week); set your own schedule

Working during business hours - Monday to Friday initially - view to extend as business grows
