Amity Health is seeking a passionate and experienced mental health professional to lead the Connections Count Program, delivering free psychological therapy and counselling to residents of Residential Aged Care Homes across the Great Southern, Midwest, Southwest, and Wheatbelt regions.
Connections Count, funded by the WA Primary Health Alliance, provides onsite, person-centred mental health care for older people experiencing mild to moderate symptoms of mental illness reducing barriers to access and enhancing wellbeing within aged care settings.
As the Program Lead, you will combine clinical expertise with operational leadership. You’ll oversee program delivery and reporting, mentor and support a small multidisciplinary team, strengthen stakeholder partnerships, and maintain your own active caseload of residents receiving psychological therapy.
About the Role
This is a meaningful opportunity to make a genuine difference. As a compassionate and experienced clinician, you will:
Lead the delivery, administration, and reporting of the Connections Count program across multiple regions.
Manage budgets, performance data, and compliance with funding agreements.
Deliver evidence-based psychological therapy and assessments for aged care residents.
Provide supervision, guidance, and professional support to team members.
Build and sustain strong relationships with RACF staff, GPs, and community stakeholders.
This is a part-time position of 4 full days per week (or 60.40 hours) per fortnight. Frequent travel will be required to the regional communities we serve. Annual renumeration is pro-rata to the part time appointment.
About You
We’re looking for a team-oriented leader who builds strong relationships and keeps both clients and colleagues at the heart of everything they do. You will have:
Tertiary qualification in Social Work, Psychology, Mental Health Nursing, or a related discipline.
Current AHPRA registration.
Demonstrated experience in mental health or aged care program coordination and service delivery.
Strong clinical skills in assessment, intervention, and case management.
Proven leadership and mentoring experience within multidisciplinary teams.
Current National Police Clearance and WA Driver’s Licence.
