The Team:

The Tasmanian Health Service delivers the Transition Care Program (TCP), a short-term community-based service supporting older Tasmanian’s after a hospital stay. Our multidisciplinary team provides person-centred care to help clients regain their independence and achieve their recovery goals. The Transition Care Program team partners with key stakeholders to deliver excellence in healthcare.

The Role:

The Clinical Nurse Specialist (CNS) plays a vital role in the delivery of the Transition Care Program (TCP), overseeing triage of referrals, care planning, and coordination of consumer care within a dynamic multidisciplinary team. The CNS also actively contributes to compliance with relevant legislation and drives continuous quality improvement initiatives.

You will need:

  1. Highly developed clinical nursing knowledge, skills, and judgement with experience providing evidence-based primary health care to older people with age-related needs or cognitive impairment.
  2. Ability to work in a leadership partnership with the Nurse Manager to maintain a strong professional environment by assisting with planning and coordination of staff and resources.
  3. Demonstrated ability to work autonomously and as part of a multidisciplinary team to provide effective and efficient client care and discharge planning within established operational frameworks and using a range of information technologies

Details of appointment:

Multiple permanent, full time or part time, day worker position, working up to 76 hours per fortnight, commencing as soon as possible.

*notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary: $106,570 to $112,440 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.

In addition, the following allowances calculated on the salaried incremental point may be available:

  • Post Graduate Allowance of up to 10 % based on qualification type

Eligibility:

Successful applicants will be required to meet the essential criteria.

Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse.

Current Tasmanian Working with Vulnerable People Registration - NDIS endorsed

Current Driver’s Licence

Applicants should note the following criteria are desirable:

Relevant post registration nursing experience e.g. Sub-Acute/Medical/Community/Aged Care.

Highly developed understanding of information technology including clinical systems, applications relevant to risk management reporting, or as required for the role and/or service.

Post Graduate Qualification (or working towards) in relevant area.
