Our Nurses are responsible for specialist community and primary health nursing services that focuses on the prevention, early detection, and management of health problems for Aboriginal and Torres Strait Islander people in line with Galangoor model of care.
In the Cancer & Palliative Care role Nurses provide expert clinical, psychosocial, and emotional support to patients and their families throughout the entire cancer journey. Their role includes educating patients about treatments, providing emotional support, advocating for patients' needs, and collaborating with other healthcare professionals to ensure continuity and a high quality of care.
They provide care coordination within a multidisciplinary team of GPs, Aboriginal Health Workers/Practitioners, social workers, and other allied health professionals. They also empower patients and families through education, support, and advocacy, aiming to improve quality of life and support patient dignity.

Salary Package – Competitive base salary + 10.5% super + salary sacrifice ( $15k+) + 5 weeks annual leave
Key Duties include:
Conducting triage assessments, gather information, assignment of clients, taking of medical and social history as per nKPI
Completion of preliminary procedures and routine surveillance services to clients aligning with nKPI data collection and within SOP and competencies.
Vital Signs, BGLs, ECGs, Urinalysis
Hearing checks
Retinal Eye Scanning
Wound management
Point of Care {POC} testing e.g., HbA1C, ACR, Hb
Conduct adult and childhood immunisations as per Handbook and National Immunisation schedule, ensuring informed consent and compliance with Galangoor protocols. Including Cold chain management and vaccine stock management.
Attend to home visits
Medication management – poly pharmacy identification and follow up regarding medication reviews {pharmacy and home}
Management of chronic disease, surveillance, gathering information, care planning, intervention and follow up and evaluation {DM, CVD, CKD, Respiratory, Musculoskeletal and Mental Health conditions – developmental and degenerative}
On a daily basis, review, action, and monitor follow up of ALL clients recalls {tests, results and referrals} as per policy Liaise / coordinate treatment, care, referral and follow up with internal and external stakeholders
Actively regularly use PenCS data to identify gaps in clinical care, prioritise, plan and implement activities within a multidisciplinary team to improve data {PIPQI}
Provision of comprehensive health promotion practices and education.
Provide advocacy and support for clients
Adopt, maintain and demonstrate best practice in all infection control activities.
Contemporaneously documenting and record keeping relevant and complete in a timely manner {including scanning of clinical documents}
Maintain consults rooms, and all clinical areas ensuring, fully stocked, clean, tidy, and equipment is clean and in working order.
Utilise, check and maintain all stock as per imprest list
Complete, delegate and manage ALL essentials of care checks {daily, weekly and monthly}
Attend to home visits with doctor if required
Adherence to the RACGP standards for general practices (5th Edition)
Maintain an AGPAL “accreditation ready” workplace and clinical environment.
Work with all Team members to oversee case management and care coordination for patients and their families.
Key Requirements include:
Current Registration with AHPRA & licensed to practice as a Registered Nurse.
Relevant clinical experience and recency of practice with commitment to evidence-based outcomes
A demonstrated understanding of the needs of Aboriginal and or Torres Strait Islander Community Controlled Health Services (ACCHS) and interest in comprehensive primary health care nursing within the Galangoor Duwalami Model of Care.
Responsive to the needs of Aboriginal and Torres Strait Islander people and has received or willing to attend training in cultural reflective practice.
Excellent communication skills with patients, their families and with the interdisciplinary team
Commitment to Professional Development of self and others to encourage lifelong learning.
Ability to plan, prioritise and time manage.
Utilises available resources effectively and efficiently for the delivery of evidence-based nursing health care.
Current Cardiopulmonary Resuscitation (annual), and First Aid Training (every 3 years).
Current Class C driver licence.
Satisfactory Police Check.
Evidence of immunisation against vaccine preventable diseases
Desirable:
Membership of appropriate professional body
Experience working as a Practice Nurse in a primary health care setting
Experience working in an Aboriginal and Torres Strait Islander Health Service and/or working with Aboriginal and Torres Strait Islander people.
Experience in Phlebotomy and/or IV cannulation (or willingness to work towards developing this skill)
Experience in Chronic Disease Management e.g., care coordination, chronic disease self-management, preparation of GPMPs (Medicare Item 732)
Experience in Cancer & Palliative Care
QLD Immunisation Program Nurse (IPN) certification
