The Role:
An exciting opportunity exists for enthusiastic, dynamic and innovative Grade 1 or 2 Social Workers to join the Northern Health Social Work Casual Bank.
This role requires the successful Social Workers to provide comprehensive psychosocial assessments, interventions and discharge planning to patients and their families on whichever ward they are allocated to. Casual shifts may be available at acute or sub-acute sites within Northern Health.
To be successful in this role you will have:
- Highly developed interpersonal, practical negotiation, conflict resolution and communication skills.
- Excellent organization skills, yet be flexible, to enable you to meet tight deadlines.
- Excellent and concise documentation skills.
- A demonstrated capacity to work as a team player both within the multi-disciplinary team and the Acute Social Work team and be an excellent communicator who is comfortable engaging productively with staff at all levels of the health service.
- Significant experience in undertaking comprehensive psychosocial assessments, intervention, discharge planning of patients within a hospital inpatient setting
- A good working knowledge of working with seriously ill patients, some of whom have challenging behaviours and cognitive issues.
- Proven ability to work independently and collaboratively with the multi-disciplinary team.
- A good working knowledge of and proven ability to access appropriate supports for and work with various sectors (eg. RACF, TCP programs, ACAS, housing services) to arrange appropriate discharge plans
- Responsible for practicing at an Intermediate Practice Level in accordance with the Family Violence Multi-Agency Risk Assessment & Management (MARAM) Framework.
- A commitment to self-initiated continuing professional development.
