  • Base Salary: $100,624 - to $131,657 p.a.

Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory.

ABOUT THE ROLE

The Population Health Clinical Leads will support the strategic and operational work of the DDHS Integrated Clinical Services team, building clinical capacity across priority program areas including blood borne virus (BBV) and sexually transmitted infections (STI) health, Rheumatic Heart Disease (RHD) and Health Promotion.

A major focus of this position will be leading coordination of one of the clinical capacity building portfolios and supporting the population health team through input and support with other portfolios which may include but are not limited to: chronic hepatitis B (CHB) care, STI/BBV health, Cancer Care, Childhood Vaccinations and RHD.

WHAT YOU WILL DO

  • Support the development, coordination, and implementation of DDHS population health programs aligned to national, NT, and local strategic priorities.
  • Work across key priority areas including but not limited to: STI/BBV, RHD, TIS, health promotion and prevention.
  • Contribute to coordination of staff and community consultation for health education and clinic quality improvement initiatives.
  • Coordinate the integration of the portfolio project into DDHS models of care, through supporting DDHS to streamline clinical pathways, activate recalls and set care plans for people with the specific portfolios' condition.
  • Develop and action a communication and data quality improvement plan for specific portfolio.
  • Work with the Health Education and Engagement lead in coordinating health education and promotion events
  • Participate in cross-cultural education as well as assist and participate in staff induction and orientation.

SELECTION CRITERIA

Essential

  • Registered as an Aboriginal Health Practitioner or Nurse with AHPRA, with proven clinical experience.
  • Experienced in clinical practice, community health education, and coordinating or delivering primary health programs.
  • Demonstrated skills in quality improvement activities and in providing training or education to clinical staff.
  • Demonstrated communication, negotiation, and conflict resolution skills, able to work effectively with diverse cultural groups.
  • Demonstrated understanding of population health principles in Indigenous settings, and high awareness of Aboriginal culture, history, and issues affecting Aboriginal and Torres Strait Islander peoples.
  • Proficient in data improvement, analysis for outcomes measurement, report writing, and maintaining confidentiality.
  • High-level computer skills, including Microsoft applications and clinical practice software.

Desirable

  • Experience working in an Aboriginal Community Controlled Health Service or similar environment.
  • Demonstrated experience in facilitating and participating in interagency forums.
