About the role
As a Case Manager, you will join a growing team that provides line management to Senior Support Workers and Support Workers. Your role will be to ensure that the case management aspects of Ausadvent Care’s operations are performed in an ethical, person-centred manner, consistent with industry standards.

You will prepare and forecast participant budgets in line with Service Agreements, while also promoting environments that support the safety, independence, growth, health, and well-being of our participants. Additionally, you will oversee case plans and risk assessments and ensure that staff are inducted and trained in accordance with those plans.

This is a full-time hybrid position, with the option to work from home and the office. If you prefer, you can also work from the office five days a week. As part of this role, you will also be required to participate in the On-Call roster to support urgent matters outside standard hours.

Key responsibilities include:

Completing the onboarding process for new participants

Providing training and mentoring to staff supporting participants

Liaising with NDIS participants, local area coordinators, support coordinators, medical practitioners, and advocates to assist participants in implementing their NDIS plans

Developing and managing participant service schedules, including service planning, confirmations, and cancellations

Preparing and transmitting service schedules to support professionals

Managing participant medication and behaviour reporting procedures

Assisting with the recruitment and retention of frontline staff

Participating in the On-Call schedule as part of the Case Management team

Essential criteria:

Industry-recognised qualifications or significant demonstrated experience

Strong communication and interpersonal skills, with the ability to work effectively with clients, families, community agencies, and other professionals

A genuine desire to provide high-quality support for people with disabilities

Ability to work in a busy environment both independently and as part of a team

Understanding of privacy and confidentiality requirements

Valid Working with Children Check, NDIS Worker Screening, and National Criminal Record Clearance

Experience in complex, solution-based care planning for participants with high needs

Demonstrated experience in support staff management or a supervisory role

Strong understanding of the National Disability Insurance Scheme (NDIS)

Experience with intake and assessment of participants

Demonstrated ability to work toward targets and participant outcomes
