Position Overview

Your Hart 2 Hart is seeking a passionate and skilled Occupational Therapist to join our multidisciplinary team. This role focuses on improving clients’ mental health, cognitive abilities, and psychosocial functioning through client-centred therapeutic interventions. You will collaborate closely with psychologists, social workers, and other professionals to deliver integrated care tailored to each individual’s needs.

Key Responsibilities

Assess clients’ mental health, cognitive functioning, and psychosocial needs using evidence-based tools.

Develop and implement personalised therapy plans aimed at improving clients’ mental wellness, cognitive skills, and social engagement.

Collaborate effectively with a multidisciplinary team to coordinate holistic interventions and share insights.

Facilitate client goal-setting, progress monitoring, and outcome evaluations.

Advocate for clients’ wellbeing and support psychosocial adjustment strategies.

Document all assessments, plans, and client communications accurately and timely.

Participate in team meetings, case conferences, and professional development activities.

Qualifications & Requirements

Current registration with AHPRA as an Occupational Therapist.

Tertiary qualification in Occupational Therapy from a recognised institution.

Experience working in mental health, cognitive rehabilitation, or psychosocial services preferred.

Strong interpersonal skills and emotional intelligence to work with diverse clients and multidisciplinary teams.

Ability to adapt communication styles and collaborate effectively across professional disciplines.
