About the role
We are seeking a dedicated and experienced Support Coordinator to join our team in Rockhampton for a permanent full-time role. Initial training will require you to be in the office for the first 2 months. After completing training, flexible hybrid or remote working arrangements may be discussed.
As a Support Coordinator you’ll empower individuals with disabilities to navigate the NDIS and connect with the right services to achieve their goals. You'll work closely with clients, families, and service providers to coordinate supports that truly make a difference.

What You’ll Do
• Support clients and families in accessing and coordinating services.
• Build relationships with internal and external stakeholders.
• Implement supports outlined in NDIS plans.
• Help clients develop skills to coordinate support and engage in the community.
• Empower clients to understand and implement their plans.
• Be responsive to client needs, especially in crises.
• Conduct regular progress reviews and report to the NDIS

What You Bring
• Relevant industry experience or qualification in disability services, social work or similar
• Understanding or knowledge of the NDIS
• Familiarity with psychosocial disabilities (advantage)
• Strong communication skills
• Demonstrable problem-solving skills

Essential Requirements

• Valid driver’s licence
• NDIS Worker Screening Check
• QLD Blue card
• NDIS Orientation Module: “Quality, Safety & You”
• Australian working rights (with no restrictions)
