About the Role

Provide outreach nursing care with a team in community settings for people experiencing homelessness or poverty (community centres, housing services, shelters).

Offer preventative care, health education, wound management, medication support, Hep C testing, and social referrals to health and community services.

Case management – Coordinate care and local referrals, link clients with appropriate services, and advocate for their health needs.

Work closely with community organisations, hospitals and other health professionals.

Participate in education, advocacy, and health promotion to improve access to care.

About You

✅ Enrolled or Registered Nurse (AHPRA) with experience in primary health, outreach, or community care.

✅ Passionate about social justice and supporting people experiencing disadvantage.

✅ Able to work autonomously and problem-solve in dynamic environments.

✅ Current driver’s licence and access to a reliable vehicle (travel is required for local outreach work).
