At Amity Health, we believe that strong connections with community are the foundations of quality health and wellbeing outcomes. Our Integrated Team Care (ITC) program is hiring an Aboriginal Outreach Worker to work alongside clients and the community.

About Us

Amity Health is a well-respected not-for-profit organisation, providing regional and rural people with the best possible access to health and social, community services.

We work in partnership with government, non-government agencies and health service providers to offer support, advocacy, leadership and coordination in the delivery and promotion of primary health care to the Great Southern, Midwest, South-East Coastal Goldfields, Southwest and Wheatbelt regions.

About the Role

In this role, you will build and maintain strong connections with local Aboriginal communities, encouraging and supporting access to health services. You will provide practical assistance to clients by helping them attend appointments and connect with GPs, allied health providers, and specialist services. Working collaboratively with community partners you will identify and address barriers that make it harder for Aboriginal people to access quality health care, while contributing to solutions that strengthen outcomes. You will also keep accurate records and support reporting to demonstrate the impact of the program. This role supports the Great Southern communities of Katanning, Mount Barker, Kojonup, Tambellup, and Gnowangerup, as well as the local Aboriginal community of Albany, frequent travel will be required (all travel expenses are covered).

Currently a 9-month contract can be offered, and we will extend in line with the funding arrangements as they present.

About You

You will bring experience working with diverse communities and building strong, meaningful relationships. Your knowledge of Aboriginal cultures, histories, and community perspectives will guide your approach, along with the ability to engage participants in a supportive and trauma-informed way. Strong communication and relationship-building skills with people from a wide range of backgrounds are essential. This position is a Section 50D identified role under the Equal Opportunity Act 1984 (WA) and is therefore open only to applicants of Aboriginal and/or Torres Strait Islander heritage. To be successful, you will also need a current National Police Clearance (within the last three months) and a valid Australian driver’s licence.
