Your new role:
As a Nurse Navigator for First Nations People, you will provide specialised clinical expertise, education, and care coordination to ensure culturally safe, patient-centred journeys across complex health systems. This role is central to improving outcomes and building trust with community.
Key responsibilities include:
- Coordinating care across primary, secondary, and tertiary services to reduce fragmentation.
- Supporting patients, carers, and families with health education and culturally appropriate care.
- Partnering with Aboriginal and Torres Strait Islander health services, community groups, and local elders.
This is an identified vacancy with a genuine occupational requirement for the position to be filled by a candidate indigenous to the Aboriginal or Torres Strait Islander Community. Candidates not meeting this criteria will not be considered.
About you:
We are seeking a motivated and culturally grounded nursing leader who is committed to advancing health equity for First Nations People. You will bring advanced clinical skills, cultural expertise, and the ability to build strong partnerships across care settings.
You will demonstrate:
- Current NMBA registration and extensive nursing experience, ideally with First Nations health.
- Ability to build trust and collaborate with patients, families, and community partners.
- Commitment to evidence-based care, cultural safety, and improving patient outcomes.
