About the Role
We’re currently seeking a Discharge Case Coordinator for a full-time, 12-month max-term maternity leave cover role. In this position, you’ll play a key role in supporting individuals with complex needs as they transition from acute care back into the community.

Working as part of a multidisciplinary team, you’ll help ensure hospital discharges are safe, timely, and person- centered. You’ll collaborate closely with hospital staff, participants, families, and service providers to develop tailored discharge plans and coordinate the essential supports needed for a successful return to community living.
This is a remote role, with regular travel required to visit clients located in Robina and Southport, as well as the opportunity to work from the Eight Mile Plains office at least once a fortnight—with flexibility depending on your individual circumstances.

What You’ll Do
• Coordinate safe and person-centred discharges for individuals with complex needs
• Develop and implement tailored discharge plans in collaboration with hospital teams and families
• Act as the key point of contact between hospitals, participants, NDIS providers, and internal teams
• Identify barriers to discharge and proactively engage stakeholders to resolve them
• Ensure smooth handovers to Support Coordinators and community-based services
• Educate and empower participants and families about next steps and available supports
• Follow up post-discharge to ensure continuity of care and address any emerging issues

What You Bring
• Background in hospital discharge planning, case coordination, support coordination, or a similar role
• Knowledge of the NDIS, disability, and/or aged care systems
• Strong stakeholder engagement and communication skills
• High attention to detail and ability to manage multiple priorities
• Confidence working independently and remotely
• A calm, solutions-focused approach to complex situations

Essential Requirements
• Qualifications in community services, health, social work, nursing, or related field (or equivalent experience)
• Current Open Driver’s Licence
• NDIS Worker Screening Check and QLD Blue Card
• Full Australian working rights
