About the Opportunity
We are looking for a Transitional / Nurse Practitioner to join us in this engaging, stimulating, and fulfilling role, with dynamic practice incorporating the application of high-level knowledge and skills in extended practice across stable, unpredictable and complex situations. As a Diabetes Nurse Practitioner you will be responsible for:
Utilise the best available evidence to guide the treatment of patients in consultation with their general practitioners and specialist medical officers.

- Manage a broad range of acute and chronic presentations and manage referrals to facilitate integrated care across disciplines or teams
- Practice autonomously initiating and managing the care and/or monitoring the ongoing therapy of patients with diseases or disorders within the scope of practice by providing effective pharmacological and other evidence based interventions.
- Providing leadership in preventative health programs and patient education in various settings.
About You
Our ideal candidate will demonstrate:
- Current authority to Practice as a Registered Nurse / Midwife with the Australian Health Practitioners Regulation Agency (AHPRA) with equivalent of three years Fulltime experience in an advanced nursing role in Diabetes.
- Authority to practice as a Nurse Practitioner or demonstrated evidence of eligibility to obtain authorisation within 2 years.
- An ability to work autonomously and within a multidisciplinary team and across acute and community settings.
- Highly developed written and verbal communication skills with proven interpersonal and negotiation skills, inclusive of highly developed skills in computer literacy with word processing and database programs.
- An ability to provide leadership along with extensive expertise in teaching and education and evidence of participation in research and quality activities related to Diabetes.
- Evidence of a current Class C drivers licence and ability and willingness to travel for work purposes. This may involve driving long distances within the MLHD.
