ABOUT THE ROLE
We are seeking a compassionate and motivated Senior Group Facilitator to join our Fraser Coast Social Support Services (FCSSS) team. Funded under the Commonwealth Home Support Program (CHSP), this service assists older people—including those from culturally and linguistically diverse backgrounds—to feel socially included and supported in their independence.
As Senior Group Facilitator, you will:
Coordinate and deliver structured, group-based activities using a Wellness and Reablement approach.
Observe and report on client wellbeing, ensuring changes are documented and communicated.
Liaise with managers, team leaders, families, health professionals, and service providers to ensure high-quality, client-centred care.
Supervise and support volunteers in group activity delivery.
Plan, promote, and implement programs that meet CHSP guidelines and service agreements.
This role offers the chance to make a meaningful impact in the lives of older people, ensuring they remain engaged, connected, and empowered.
ABOUT YOU
You are passionate about supporting older people to maintain independence and connection in the community. You are organised, proactive, and confident in leading group activities and working collaboratively with a team.
To be successful, you will have:
Certificate III in Individual Support, Certificate IV in Leisure and Health, or equivalent qualification.
Current First Aid Certificate.
Current QLD Driver’s Licence.
Working with Children Card and valid Federal Police Check.
Prescribed doses of Covid-19 vaccination in line with Queensland Health requirements.
Strong communication skills and the ability to liaise effectively with clients, families, and service providers.
Knowledge of client-centred care, wellness, and reablement approaches.
Experience in aged care or group program facilitation will be highly regarded.
