Who We Are
Bridges Health & Community Care Ltd. (BHCC) is a purpose-driven, community-based organisation delivering life-changing programs across regional and rural Queensland since 1997. We’ve supported thousands of people to navigate mental illness, addiction, disadvantage and complex needs — moving from isolation to connection, from stigma to purpose.
Our work is grounded in recovery-oriented practice, “No Wrong Door” principles, and a deep commitment to lived experience, cultural respect and inclusion. We are proud to be launching the Staying on Track (SoT) program, supporting young people aged 10–17 who are exiting detention, helping them safely reintegrate into their families and communities, with wraparound support for education, training, employment, cultural identity, and resilience.
About the Opportunity
As a Youth Mentor, you will be a steady, reliable presence for young people during one of the most important transitions of their lives. You’ll support them from detention into community life — providing mentoring, guidance and practical help, particularly during the first 72 hours post-release.
Key responsibilities include:
Mentoring young people across home, school, and community settings
Providing outreach, transport and support at appointments
Building cultural identity and resilience through one-on-one support
Offering continuity and stability during high-risk periods
About You
You are committed to walking alongside young people and creating safe, trusting relationships. You’ll bring:
Certificate IV in Youth Work, Community Services or similar
Hands-on experience supporting young people with complex needs
Cultural awareness and respect for lived experience
Lived experience or cultural connection highly valued
Traditional Owners of the Wide Bay region strongly encouraged to apply
