The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.

Staying Deadly|headspace Deception Bay is part of the IUIH network and aims to promote and facilitate improvements in the mental health, social wellbeing and economic participation of young people aged 12-25 residing in the Moreon Bay region. In this young person friendly space, services are delivered across four core streams – physical and sexual health, mental health, alcohol and other drugs (AOD), and educational and vocational services.

About the Role?

Staying Deadly|headspace Deception Bay provides early intervention mental health services to young people aged 12-25 supporting them with mental health, physical health (including sexual health), alcohol and other drug services, as well as work and study support. As part of the headspace National Youth Mental Health Foundation network, it provides young people and their families with support at a crucial time in their lives and helps to get them back on track and strengthen their ability to manage their mental health in the future. Staying Deadly|headspace Care Coordinators provide outcome focused health, social and practical support to Aboriginal and Torres Strait Islander young people aged 12 to 25 years. Working within a multidisciplinary team to support young people aged 12 to 25 years with mild to moderate mental health needs to ensure they are well supported in the community through holistic and wrap around primary health and mental health care. The role will manage a caseload of young people aged 12 to 25 with varying needs, to empower and foster strength, resilience and well-being through participation in intensive case management, planning and coordination.

Day to Day Duties Include:

  • Engage with the community to raise awareness of Staying Deadly|headspace, promote mental health and wellbeing and reduce stigma. Developing and maintaining support plans in conjunction with clients and relevant service providers.
  • Work with clients in a trauma informed, recovery and/or strengths-based approach to formulate, implement and review well-being plans including a comprehensive assessment of need and safety planning.
  • Participate in care coordination activities to fulfil needs of assigned clients transition care plans, including coordination meetings with relevant external stakeholders.
  • Work with Centre Manager and Care Coordinators to develop and deliver community awareness and engagement strategies designed to increase engagement of priority groups of young people, particularly through schools, in line with the Centres community engagement and awareness plan.

About You?

  • Current C Class Drivers Licence (QLD)
  • Current Criminal History Check
  • Valid Blue Card
  • Current CPR and First Aid Certificate
  • Certificate IV or Higher in Aboriginal and/or Torres Strait Primary Health or willing to complete.

Desirable, but not mandatory:

  • Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
