We're now looking for a Liaison Officer—a communicator, coordinator, and care-focused professional—to be the key point of contact for participants and the wider support network around them.
About the Role
This is more than an admin job—it's about people. As Liaison Officer, you'll be on the ground and in the loop: meeting participants, visiting their homes, checking on their wellbeing, and making sure everyone involved in their care is informed, coordinated, and supported. You'll liaise with support workers, families, health professionals, and NDIS stakeholders, making sure nothing falls through the cracks.
Your responsibilities will include:
Acting as the primary point of contact for participants and their care teams.
Visiting participant homes to check on wellbeing, living conditions, and care delivery.
Monitoring that external appointments (medical, therapy, etc.) are tracked and followed up.
Collaborating with office staff to generate and deliver regular progress reports.
Attending or facilitating stakeholder meetings and ensuring clear communication between all parties.
Ensuring incident reports and support worker notes are submitted and actioned appropriately.
Getting to know the support workers delivering services to build a strong, cohesive care team.
Keeping participant files up-to-date and ensuring NDIS compliance.
Being part of the on-call rotation for urgent matters (shared across the team).
You'll have access to a company vehicle for travel between participant homes and appointments.
About You
You're a natural communicator and connector. You're confident walking into a participant's home or a meeting with allied health professionals and knowing exactly what to say—and what to listen for. You thrive on being organised, responsible, and supportive. You understand how important it is to deliver services with both compassion and accountability.
Candidates must hold permanent residency status or have appropriate Australian work rights to be eligible for this position.
A Working with Vulnerable People check or equivalent background screening must be completed and provided before commencement.
You'll ideally bring:
Experience in a similar role—liaison, case management, community services, or coordination.
A minimum of 2-3 years experience in liaison, case management, or coordination roles is required.
Strong knowledge of the NDIS and common medical or psychosocial conditions.
A genuine commitment to person-centred care.
Great written and verbal communication skills—able to produce professional reports and speak confidently in meetings.
Ability to build relationships with a wide range of people—participants, families, support workers, and professionals.
A current driver's licence and willingness to travel locally and to Toowoomba
Qualifications in Social Work, Community Services, Communications, Health, or similar—or equivalent experience.
