Key Responsibilities:

Provide High-Quality Nursing Care: Deliver compassionate, patient-centred care in a general practice setting, ensuring patients feel valued and supported.

Clinical Support: Assist GPs with tasks such as immunisations, wound care, chronic disease management, and health assessments.

Care Coordination: Contribute to the development and implementation of care plans to support patient health needs.

Health Education: Educate patients on preventive care, treatment plans, and overall well-being.

Patient Records: Maintain accurate and up-to-date electronic health records, ensuring seamless coordination of care.

About You:

Registered Nurse: You hold a current registration with AHPRA as a Registered Nurse.

Experienced in General Practice: You bring experience in a primary care setting and have a strong understanding of chronic disease management and clinical assessments.

Great Communicator: You have excellent communication skills and enjoy building rapport with patients and team members alike.

Team Player with Independence: You can work autonomously and as part of a multidisciplinary team, managing patient care with confidence.
