With over 25 years’ experience, we offer a wide range of services including Home Care Packages (HCP), CHSP programs, DVA community nursing, Transitional Care, Veterans’ Home Care (VHC), and permanent care options.

We’re known for our culturally inclusive approach, strong community ties, and supportive, collaborative team. With reforms reshaping aged care, we’re expanding our nursing team to help lead the way in delivering the future of Support at Home for clients with clinical needs.

The Role
As a Clinical Care Partner, you’ll provide both nursing care and care management for older clients living at home. This is a varied role that combines clinical expertise with building trusted relationships. You’ll split your time between office work and visiting clients in the community (with a company car), ensuring care is person-centred, safe, and empowering.

What you’ll do:

Lead client intake and onboarding, conduct assessments, and develop and review care plans and budgets.

Provide nursing services such as wound care, medication support, catheter care, and dementia strategies.

Support and mentor care workers, ensuring seamless delivery of care and continuity for clients.

Work closely with families, representatives, the care team, internal and external stakeholders, health professionals, and the Aged Care Quality and Safety Commission to achieve the best outcomes for clients.

Maintain full compliance with the Aged Care Act, program funding guidelines, Quality Standards, and organisational policies.

About You

We’re looking for a registered nurse who is compassionate, organised, and thrives on autonomy as well as teamwork.

Essential:

AHPRA registration – Registered or Enrolled Nurse

Valid Driver Licence (willingness to drive to client homes, company car provided)

National Police Check (or willingness to obtain)

Full Australian work rights.

Desirable:

Experience in community or aged care, particularly managing clients with complex needs under Home Care Packages (HCP)

Strong assessment, planning, and clinical decision-making skills

Cultural awareness and ability to connect with diverse clients

Confident with digital care systems and Microsoft Office.
