About the role
Temporary part time opportunity to be involved in developing and implementing transition of care services for cariology and cardiothoracic patients at the Royal Brisbane and Women's Hospital in the context of the Research Project “Reducing hospital re-admission for high-risk CARDiology patients (RECARD)”.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Deliver specialised pharmacy clinical and consultancy services to cardiology patients in accordance with accepted professional practices, legal and ethical requirements with a high degree of independence.
- Provide professional leadership and responsibility for development of Pharmacy clinical practices, procedures and protocols.
- Provide clinical advice and direction to the multidisciplinary healthcare team to ensure effective service delivery and quality outcomes applying change management, conflict resolution and negotiation skills where necessary.
- Foster an interprofessional team environment to ensure achievement of RECARD and departmental objectives including providing constructive and effective feedback which achieves solutions.
