Position: APS6 Clinical Care Coordinator
Location: Darwin, NT
Start date: As soon as possible
Duration: Approximate 2-year contract with potential to extend
Pay rate: $55.00 - $65.00 per hour plus 12% superannuation
ABOUT THE ROLE As an APS6 Clinical Care Coordinator, you will play a pivotal role in supporting the veteran and Defence community through short-term, goal-oriented clinical case management. Your primary focus will be on assisting clients with complex health and psychosocial needs, removing barriers to accessing mental health services, and ensuring engagement in care.
Working within a national counselling service, you will collaborate closely with internal teams and external stakeholders to deliver trauma-informed, evidence-based support that promotes recovery and wellbeing.
KEY RESPONSIBILITIES
- Provide clinical services including risk management, brief intervention, care coordination, and referrals for clients with complex needs.
- Provide high quality clinical services including risk assessment and intervention, referral, evidence-based individual, couple and family counselling, and case management to eligible clients in order to achieve outcomes identified in care planning.
- Maintain accurate and confidential case notes, care plans, risk assessments, and progress reports.
- Build strong relationships with Defence, ex-service organisations, mental health providers, and community agencies to coordinate client care.
- Support clients using strategies such as psychoeducation, problem-solving, distress tolerance, and motivational interviewing.
- Participate in professional development, supervision, and service promotion activities.
- Contribute to quality improvement, program evaluation, and research initiatives.
- Facilitate group treatment programs and oversee quality assurance processes for contracted clinicians.
SKILLS AND EXPERIENCE
- Strong interpersonal skills with experience in trauma-informed, client-centred practice.
- Ability to complete thorough intake assessments and risk management plans.
- Knowledge of clinical case management practices and risk escalation pathways.
- Excellent written and verbal communication skills.
- Ability to respond to critical incidents calmly and effectively.
- Confident IT skills and ability to manage client information systems.
ELIGIBILITY
- Working with Vulnerable People registration OR Working with Children check.
- Psychologist, Occupational Therapist, or Registered Nurse with full AHPRA registration; OR
- Social Worker with AASW membership; OR
- Masters-level Counselling qualification with PACFA (Registered Clinical Counsellor) or ACA Level 3 or 4 membership.
- Must be an Australian Citizen.
- Ability to obtain and maintain a Baseline Vetting security clearance.
