Mudgeeraba Family Medical Centre is looking for a friendly, dedicated, efficient Chronic Disease Management Nurse to join our team

About the role

Your prime focus will be on delivering quality clinical nursing services to patients via chronic disease management but not limited to triage, vaccinations, wound care, health assessments, recall, cold chain management and sterilisation.

Skills & experience

Previous experience working in general practice (preferred)

Strong communication and time management skills

A caring and collaborative approach to patient care

AHPRA registration & able to use Best Practice Medical Software

Duties include but not limited to:

Chronic Disease Management

Health Assessments

Child Immunisations

General immunisations

Vaccine Ordering

Sterillization

Wound dressings/care

ECGs, spirometry

Ear Syringing

Triaging

Infection Control Management

Maintaining recall and reminder system

Cold chain management

Maintaining the operation of practice policies and procedures to meet agreed standards in patient care

Assistance in delivery of premium customer service and enhancing the image of the practice with health professionalism

About you:

Current AHPRA registration

Previous experience in Chronic Disease Management

Previous General Practice experience

Best Practice software knowledge (desirable)

Ability to multi-task and work in a busy environment
