The Role
Our Health Navigation team launched in August 2023 when our Hobart Wellness Hub opened its doors and expanded its services in 2024 at our Launceston Wellness Hub. Since that time, St Lukes has been helping Tasmanians at a grassroots level by providing support and direction in navigating the Tasmanian health ecosystem, empowering them to manage their health care journey with confidence.
As Health Navigation Team Leader, you will lead and support our passionate Health Navigation team, as well as actively contributing to service delivery. In addition to this, you will play a key role in building awareness of the Health Navigation program through outreach and community engagements.
To be successful in this role, you will have:
Proven experience in leadership, with experience managing small teams. Experience managing teams across multiple sites is desirable
Exceptional communication skills, with the ability to quickly build rapport with a diverse group of people and work collaboratively with a range of stakeholders
A clear understanding of the health ecosystem in Tasmania through lived experience or professional experience in a health-related field.
Relevant experience in community outreach, health navigation, case management or related role
A values based approach to Health Navigation that enables you to put the member at the centre of all activities.
Willingness to undertake intrastate travel as the role requires
