As a Case Manager at TDN, you will play a pivotal role in coordinating services for a diverse range of clients, collaborating with team members and various healthcare providers to deliver high quality care. Your primary objective will be to support recipients of Home Care Package funding who aspire to maintain their independence and live comfortably in their own homes.
Key Responsibilities
As the Case Manager, you will:
• Conduct integrated assessments to understand the unique needs of each client.
• Collaborate with a variety of healthcare professionals and service providers to coordinate an array of services.
• Establish and maintain strong relationships with key stakeholders involved in the client's services.
• Ensure that government funds are efficiently utilised within the bounds of legislation and guidelines.
What We’re Looking For
We are seeking someone who has:
• Exceptional organisational skills
• Commitment to working within a multidisciplinary team
• Proficiency in verbal and written communication
• Strong computer skills, particularly with Microsoft Suite and client management systems
• Able to self-manage and work independently
