IWA Australia is seeking a Care Partner Lead to drive and oversee the delivery of client centered case management services. The Care Partner Lead will provide high quality support by supervising and mentoring Care Partners and Home Care Employees in providing case management and services to our aged care clients.

This role requires leadership experience and the ability to guide a team in delivering flexible, culturally responsive, and individually tailored services to clients, including those with complex physical and/or behavioural needs.

Responsibilities:

As the Care Partner Lead, your main responsibilities include:

Provide day-to-day leadership, guidance, and support to Care Partners.

Conduct regular supervision, performance reviews, and mentoring sessions.

Support team development through coaching, training, and identifying learning needs.

Foster a collaborative and accountable team culture aligned with organisational values

Manage intake enquiries from potential participants, ensuring accurate information is provided and referrals are processed efficiently in line with organisational procedures

Maintain and update the client referrals spreadsheet.

Maintain and monitor participant MAC portal waitlist.

Ensure care plans and budgets are developed and reviewed in a timely manner.

Monitor service agreements and risk assessments for compliance and quality.

Assist with facilitation and support training for Care Partners and HCEs.

Attend mandatory and/or recommended training and development courses/modules as assigned by Senior Managers

Promote and market IWA services.

Eligibility Criteria:

A relevant qualification or minimum 2-year experience in client coordination and leadership

Interest and willingness to work in culturally and linguistically diverse community service environment

Drivers Licence

NDIS Clearance

Desirable criteria:

Ability to speak a second language

Please note, this position will be offered on a part-time fixed term basis for 12 months and subject to review after this date.
