Home Care Manager – Rural WA

We’re recruiting on behalf of a respected aged care and home care provider seeking an experienced Home Care Manager to lead their community care team.

The Role:

You'll be responsible for leading a team of care staff and coordinators, ensuring high-quality, person-centred services are delivered to clients living independently at home. This is a hands-on leadership role focused on people, performance, and compliance.

Key Responsibilities:

· Manage and support a team of community care workers

· Oversee service delivery aligned with Aged Care Quality Standards

· Monitor care plans, client outcomes, and team performance

· Liaise with families, health professionals, and stakeholders

· Drive quality, compliance, and operational efficiency

About You:

· Proven leadership in home/community care

· Strong knowledge of HCP/CHSP or NDIS

· Excellent communication and team management skills

· Relevant qualifications in Health, Aged Care, or Community Services

· Current police check & driver’s licence
