About the role
- Coordinate, lead and participate in projects related to patient flow and care coordination from one service location to another
- Monitoring patient wait times and estimated discharge dates through update and management of electronic patient journey board as well as identifying key 'Waiting for What' (W4W) and patient exit blockers
- Leading multidisciplinary meetings in relation to patient flow and movement, to ensure that patients are seen in a timely manner. Advising and negotiating with referrers, and escalating any bed blocks
- Work in a dynamic team leading our approach to the provision of quality care to community clients of the Palliative Care Service
- Provide evidence-based nursing care to people of all ages and cultural groups, including individuals, families and communities
- Take an extended leadership role in the coordination of nursing and health care, providing expert nursing assessment and triage to facilitate optimal health outcomes. Feeding back to staff on how to improve patient satisfaction or efficiency
About you
- Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency
- Current NSW driver's licence (Class C or above)
- Relevant post-registration qualifications and at least three years' experience working in palliative care
- Demonstrated ability to work within the interdisciplinary team at a senior level with a commitment to ongoing professional development and demonstrated effective communication and interpersonal skills
- Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice as a registered nurse with effective time management skills and ability to prioritise competing workloads
